Patient pricing: $60
(Additional plans purchased at the same time are $30 each)

• 25% off regular cleanings

• 25% off all periodontal cleanings

• 25% off exams (comp, periodic, perio)

Terms and Conditions

• Membership fee must be paid in full at the time of enrollment and before first appointment. 

• Membership expired one (1) year from the date of purchase.

• This is NOT dental insurance.

• Membership benefits cannot be combined with other discounts or insurance. 

• Unused benefits do not “roll over” to the next paid membership year. In-house payment plans are available for prearranged operative work. 

• No refunds on the purchase of this plan. 

• Plan cannot be used for services involving workman’s comp cases, other offices such as specialists, or care credit.
