Middleton Family Dentistry was established in 1983. Today it is co-owned by Dr. Mat Middleton and Dr. Shim, with a team of associate dentists across two Fort Wayne offices, the same family-style care, carried forward.

Dr. Mat Middleton and Dr. Shim own the practice. Alongside them, a team of associate dentists carries the week-in, week-out work: the cleanings, the cosmetic cases, the restorative rebuilds, and the patients who come back for decades.

Dr. Christine Lam
Associate Dentist
Chicago-raised, Midwestern University-trained DMD. Dr. Lam leads most of our Invisalign cases and works extensively in same-day CAD/CAM ceramics.
Endodontics & GentleWave
Dr. Logan Goodrich
Associate Dentist
A Huntertown native and Tufts top-of-class graduate. Dr. Goodrich handles much of our endodontic work and leads our use of the GentleWave root canal system.
General Dentistry
Dr. Benjamin Boswell
Associate Dentist
An IU School of Dentistry graduate. Dr. Ben handles the steady cadence of general dentistry: exams, fillings, crowns, and the visits that keep families on track.
Restorative & Digital Dentistry
Dr. Tulsi Soni
Associate Dentist
A Chicago-area native and Southern Illinois University School of Dental Medicine graduate. Dr. Soni joined the practice in 2026 and works across restorative, preventive, and digitally planned care.
