Middleton Family Dentistry provides dental and aesthetic services at our Fort Wayne, Indiana locations:

Our services include general and family dentistry, cosmetic dentistry, aesthetic treatments (PRF, Botox, microneedling), and preventive care. All services are subject to availability and may require consultation prior to treatment.
Appointments and Cancellations
We request at least 24 hours' notice if you need to cancel or reschedule an appointment. Failure to show for a scheduled appointment without prior notice may result in a cancellation fee.
